You’re invited to attend a Population Health Management (PHM) training session hosted by the Suffolk and north east Essex (SNEE) PHM Programme Team.
This 1.5-hour virtual session, held via Microsoft Teams, is open to anyone working in the Suffolk and north east Essex system who is interested in learning how data-driven approaches can support evidence-led commissioning and system transformation.
To ensure a more interactive and tailored experience, each session is limited to 25 participants. We encourage you to register early to secure your preferred timeslot.
